Out-of-State Breast Pump Rental Procedure

1.) Call (702)862-6435 to place your order and pay with your credit card.
2.) We will then email a Rental Agreement to you, with your information and breast pump rental cost information filled in. (See below to review rental agreement*.)
3.) Verify that all information is correct, and then print the email.
4.) Sign and Date the Rental Agreement, and send back to us via email attachment or fax to (702)562-5063.

Your breast pump (and supplies if needed) will be shipped the same day if your order is paid for AND we receive your signed Rental Agreement before 3pm PST, (M-F) or shipped the next business day if after this time.

Breast Pumps are shipped from Las Vegas, Nevada via Fedex Ground Home Delivery, or can be shipped USPS Priority Mail for an extra charge. (See below for shipping times.)

Shipping is FREE for prepaid rentals of 2 months or longer.

Flat rate shipping = $15 for one month rentals. (Continental U.S.)

Your rental period will begin on the day you receive the breast pump.

Each rental month is a full month, starting and ending on the calendar day that you received the breast pump.

After your prepaid period, your credit card will be automatically charged monthly until rented equipment is returned, as stated in Rental Agreement.

There is no security deposit required.

We clean all breast pumps between rentals - you do not have to pay a cleaning fee.

Sorry, no refunds for unused paid rental time.

Return shipping cost is the responsibility of the lessee.

Upon request, we will provide a RETURN Fedex shipping label (actual cost charged to your credit card) for shipping the pump back. You must use the same box or supply us with the new box dimensions (and weight) when requesting the return label.

All rentals require a signed Rental Agreement.
If you do not have a fax machine or scanner, you can send a fax at your local Kinkos or Office Max.
Or, if you have time to spare, you can mail your signed contract via "snail mail".

Deposit and Prepayment
The Rental Station has waived the security deposit.
The Rental Station will charge $_______ to lessee's credit card upon receipt of signed rental agreement, which is non-refundable once pump has shipped.

**Lessee is responsible for return shipping charges.
Return shipping must be through Fedex, UPS or USPS Priority Mail - NOT Parcel Post.
RETURN SHIPPING INSTRUCTIONS are included with pump.
If you would like a Return Fedex shipping label sent to you by email (actual shipping cost charged to your credit card) please call or email at least one day before you plan to ship the pump.


PRIVACY NOTICE
We do not disclose any non-public protected information about our customers or former customers to anyone, except as permitted by law. We restrict access to non-public protected information about you to our employees who need to know that information to provide products or services to you. We maintain physical, electronic, and procedural safeguards that comply with federal regulations to guard your non-public protected information.

Terms of Agreement
This Agreement for the rental of a Medela Product and carrying case (“equipment”) is made between Rental Station and the Lessee identified at top.
1. The equipment remains the property of Medela, Inc. McHenry, IL. Lessee has no rights to such equipment except as expressed in this Agreement.
2. Lessee may purchase an accessory kit. The kit becomes property of the Lessee.
3. Lessee agrees to pay the rental fees in accordance with the fee structure established shown under the rental payment plans above. The rental fee is due in advance.
4. The rental rates for the equipment and the purchase price for the kit do not include any applicable sales or use tax.
5. Lessee shall leave a security deposit with the Rental Station. (WAIVED)
6. Lessee agrees to inform the Rental Station of any change of address.
7. Lessee agrees not to move the equipment out of State without the consent of the Rental Station.
8. Lessee agrees to allow no person other than Lessee to use the equipment provided.
9. Lessee agrees to return the equipment in clean condition.
10. Lessee agrees to return the equipment in good repair. If the equipment is not in good repair, Lessee agrees to pay Rental Station a charge of $30.00.
11. This Agreement shall be construed under the laws of the State where the Rental Station is located.
12. The Lessee shall be responsible for all reasonable legal fees and other costs involved in collection of overdue amounts and/or recovery of equipment.
13. The Rental Station has the right to cancel this agreement at any time with three days notice.
14. Lessee agrees to allow Rental Station, Medela, Inc. or any agency involved in collection of overdue amounts and/or equipment to obtain a credit report on Lessee.

RENTAL STATION WARRANTS TO REPAIR OR REPLACE ANY EQUIPMENT WHICH IS OR BECOMES DEFECTIVE UNDER NORMAL USE. RENTAL STATION AND MEDELA, INC. WILL NOT BE LIABLE FOR INCIDENTAL, CONSEQUENTIAL OR ANY OTHER DAMAGES RESULTING FROM DEFECTIVE EQUIPMENT; THEIR LIABILITY IS LIMITED TO REPAIR OR REPLACEMENT OF THE EQUIPMENT. THIS WARRANTY IS GIVEN IN LIEU OF ALL OTHER WARRANTIES EXPRESSED OR IMPLIED.
